Aim: To evaluate clinical feasibility of the HRCT evaluation during 18F-FDG-PET/CT in cases of increased FDG accumulation in the lung tissue.
Material and method. A retrospective analysis of the 500 consecutive body 18F-FDG-PET/CT examination was done. Imaging protocol contained reconstruction of the 1 mm thin images of the chest with high resolution algorithm. These patients with known bronchogenic carcinoma or hematogenous metastatic spread of extrapulmonary tumor were excluded. The important lung finding was found in 34 cases. HRCT findings were correlated with findings of autopsy, biopsy, immunology, targeted therapy response or clinical follow up.

Results: The correct diagnosis according to the HRCT finding was concluded in 32 patients, incorrect in 2 another respectively. Only in five cases led the PET evaluation in correct diagnosis (15%). Overall accuracy of the lung parenchymatous diseases assessment during 18F-FDG-PET/CT including evaluation of lung HRCT reached 94% (32/34).

Conclusion: HRCT evaluation enables to improve significantly the accuracy of the PET/CT interpretation in patients with increased accumulation of FDG in lung tissue.
