SPECT/low dose CT enhanced scintigraphy detection of the spleen multiple lymphangiomasOriginal article

Aim: To present scintigraphy, SPECT and SPECT/low dose CT images of multiple spleen lymphangiomas in 35-years-old woman with history of the idiopathic thrombocytopenic purpura and multiple lymfangiomas in the Th and L vertebrae.

Method: The woman was examined for a palpable resistance and a few weeks persisting pain in the left epigastrium. The abdomen CT detected the enlarged spleen with multiple cysts. The ovoid formation (diameter 15 mm) in the spleen hilus was imaged. It was given a thought to accessory spleen in a patient with positive history of the idiopathic thrombocytopenic purpura. The static spleen scintigraphy with 99mTc-Sn-colloid was performed before splenectomy. The spleen SPECT and SPECT/low dose CT was added to improve sensitivity of the examination. The double-head multi-imaging SPECT/low dose CT scanner with large field of view was used. The scanner was fitted with low-energy parallel collimators with infra-red body contouring and a high resolution.

Results: The scintigraphy and SPECT imaged multiple spleen photopenic lesions which were fused to cystic spleen lesion on the SPECT/low dose CT. The finding was compatible with diagnosis multiple lymfangiomas in spleen. No accessory spleen was detected in spleen hilus by means of scintigraphy and SPECT. The woman underwent laparoscopic splenectomy and diagnosis of multiple spleen lymphangiomas was confirmed.

Conclusion: The scintigraphy and SPECT excluded accessory spleen in the spleen hilus and images multiple photopenic lymfangiomas in the spleen. The hybrid SPECT/low dose CT put more precisely the space localization of the photopenic/cystic lesions and their number.
