Complementary role of two-phase multidetector CT-angiography and 18F-FDG-PET in hybrid PET/CT imaging of the hepatocellular carcinomaOriginal article

Aim: To assess the impact of two-phase multidetector-row CT-angiography (MDCTA) combined with 18F-FDG-PET included in PET/CT imaging for hepatocellular carcinoma (HCC) depiction and differential diagnostic evaluation.

Method: Eleven patients (9 men, 2 women, mean age 60.9 years, range 53-76 years) with hepatocellular carcinoma underwent 18F-FDG-PET/CT among 1000 consecutive examinations. Examination protocol consisted of PET data acquisition (delayed 60 minutes after intravenous application of radiotracer) followed by MDCT data acquisition (collimation 16 × 0.75 mm, pitch 1.5) in arterial and portal venous phase after administration of 120 ml of non-ionic contrast agent.

Results: High 18F-FDG uptake characterized all cases of poorly differentiated HCC, mildly-increased 18F-FDG uptake was noted in three well-differentiated HCCs, equal 18F-FDG uptake between tumorous and non-tumorous tissue was present in three well-differentiated tumors. On two-phase MDCTA, hypervascularization was found in all cases of HCC and invasion into portal vein in four cases. Distant metastatic involvement showed high radiotracer uptake both in poorly and well-differentiated HCCs on PET.

Conclusion: During hybrid PET/CT imaging, two-phase multidetector-row CT (MDCT) in arterial and portal venous phase makes the depiction of HCC in liver parenchyma more accurate and enables detection of intravascular spread of the tumor. The contribution of PET is in the evaluation of HCC differentiation and in depiction of distant metastases.
