During difficult colonoscopic polypectomy intestine perforation occurred with following findings: pneumoperitoneum, pneumoretroperitoneum, pneumomediastinum, subcutaneous emphysema and pneumothorax. Spread of air into peritoneal cavity, retroperitoneum, mediastinum, and subcutaneously can be simply explained. Because there is no direct way to pleural cavity, insult of pleura had to occur. Pneumothorax is possible complication of colonoscopy which had been described in few case-reports.
