The authors review the history, indications, technical problems, complications, logistics, long-term results and technical developments of endovascular treatment of acute intracranial aneurysms. Competent vascular neurosurgeons and interventional neuroradiologists should collaborate in population-based neurovascular centres, discussing and tailoring individual treatment procedures. The morphological, anatomical and clinical (neurological deficit) aspects are the main areas discussed. Centralized treatment of patients with SAH may eliminate other problems, such as skills, self-confidence, availability and treatment expenses.

Keywords: intracranial aneurysms, subarachnoid haemorhage, mortality, outcome, endovascular therapy
