Aim: Thirteen patients with intracranial lipomas were detected in a retrospective study. A detailed MRI examination of patients with intracranial lipomas was performed - the localization of the lipoma, the patient's clinical symptoms and related congenital brain malformation were assessed.

Methods: All patients had undergone MR imaging and two patients were also scanned using CT.

Results: The predominant localization of the lipomas was close to the midline structures, such as the areas around the corpus callosum, the quadrigeminal cistern and other midline structures. The association with other brain malformations was much less frequent than previously reported: dysgenesis of the corpus callosum was only observed in three cases.

Conclusion: The lipoma was an incidental finding not related to the clinical symptomatology and with only a minor association of congenital brain malformations. It is necessary to differentiate the lipoma from an epidermoid, dermoid and haemorrhage at the methemoglobine stage when interpreting the MRI.
