Parallel to the noinvasive imaging of the carotic arteries with doppler-ultrasound, CT-angiography of the carotids is developing since early nineties of the twenties century. Although the Z-axis resolution remained only 2 to 3 mm in the early years, the sensitivity and specificity reached sufficient values in detection carotic bifurcation stenoses. During introduction of the multidetector-row systems with enabled submilimeter isotropic resolution increased rapidly the image quality in the carotic bifurcation and also increased the optimum quality scanable range. The assessments, including soft-plaque-in-risk detection, the aortic arch branches, vertebral arteries and carotic arteries in carotic channel and arteries forming circle of Willis are an integrall part of the carotic CT-angiography evaluation. Further improvements of cervical arteries imaging as detectability of the blood volume perfusion deficits calculated from CT-angiography data sets or dual-energy scannig introducing the more complex diagnostic view in diseases of the cervical arteries are discussed.
