Sentinel lymph node detection in breast cancer: peritumoral, subdermal, or combined application of radiopharmaceutical?Original article

Thirty four patients (pts.) with breast carcinoma underwent sentinel lymph node (SLN) scintigraphy. In all pts. peritumoral and subdermal administrations of 99mTc-nanocolloid were performed separately. The SLNs were displayed in the 32/34 pts. (internal mammary SLNs in 3 pts.) just after peritumoral injections (inj.). Additional subdermal inj. led to imaging of further axillary lymph nodes in 10 pts., but no internal mammary SLNs were detected. In 2 pts. the SLNs were detected using a subdermal inj. only. It can be concluded that peritumoral inj. of nanocolloids enabled to image SLNs in the majority of the pts. Peritumoral and subdermal administrations of nanocolloids revealed in some pts. different lymphatic pathways. Subdermal inj. should be used in pts. when accumulation of the radiopharmaceutical in SLNs after peritumoral inj. is insufficient for subsequent radioguided SLN biopsy. Simultaneous peritumoral and subdermal inj. of radiopharmaceutical should be applied in case of impossibility to use more time-consuming method with separately performed peritumoral and subdermal administrations.
