Radiologic evaluation of spine sagittal profile in surgically treated scoliotic deformitiesOriginal article

Aim: The study focuses on the four radiologic parameters describing sagittal stability of a spine in patients with surgically treated scoliotic deformities.

Method: In a prospective study we observed the changes of lordosis, kyphosis (measured by Cobb's angle), T9 and sagittal tilt angle (measurement by Duval-Beaupére) on lateral long-format X-Ray projections of spine. The study was made in a group of 76 patients (50 idiopathic and 20 neuromuscular scoliotic curves) examined at Radiological Clinic and treated surgically at Orthopedical clinic of The Faculty Hospital Brno by means of posterior correction and stabilization. The measurement was realized pre-operative, immediately after surgery, 4 months after surgery and further in 6 months interval. For data evaluation parametric and non-parametric statistical analysis was used.

Results: We found significant difference in pre-operative thoracic kyphosis angle comparing group of patients with idiopathic and neuromuscular scoliosis. Statistically significant was also difference in lumbar lordosis angle and sagittal tilt angle comparing pre- and post-operative state in the subgroup of patients with neuromuscular scoliosis.

Conclusion: Surgical treatment of idiopathic scoliosis does not cause significant changes in sagittal plane. Conversely, surgical treatment of neuromuscular scoliotic curves lead to significant change of lumbar lordosis and sagittal tilt angle in correlation with generally favorable clinical impact of surgery on sagittal stability of spine. Therefore these parameters can be interpreted as valuable objective indicator suitable for assesment of surgical treatment impact on sagittal profile of a spine ant its further progression.
