Late endoleak type II after stentgraft implantation for subrenal aortic abdominal aneurysmOriginal article

Aim: To evaluate the incidence of late endoleak type II and reinterventions after endovascular treatment of subrenal abdominal aortic aneurysm with stentgraft.
Method: From April 1996 to December 2012 we treated 288 patients with abdominal aortic aneurysm endovascularly with stentgraft implantation at our department. There were 248 men and 40 women aged from 49 to 91 years. The average age was 69 years. 218 patients were followed for at least a year after treatment. In these patients incidence of late endoleak type II and reinterventions for this endoleak were evaluated.
Results: In a group of 218 patients 33 (15 %) late endoleaks were found. In 3 (9 %) patients aneurysm sac was enlarger (in two 1 year, in one 10 years after treatment). Endoleaks II with sac enlargement were treated by ligation in one patient and by embolization of source in two patients. Sac size was stationary after treatment in all patients.
Conclusion: Endovascular treatment of abdominal aortic aneurysm is associated with the occurrence of late endoleak type II in our group in 15 %. Endoleak II is indicated for treatment if aneurysm sac grows, it is in 9 %. Translumbal sac puncture with trombin administration in the site of endoleak should be indicated as first treatment method.
