Prophylactic occlusion of internal iliac arteries in patients with abnormal inherent or inserted placentaOriginal article

Aim: To describe the technique of prophylactic occlusion of internal iliac arteries in pregnant patients with pathologic placenta during Caesarean section and evaluate our first experience.

Method: From November 2015 to January 2016 we performed a prophylactic occlusion of the internal iliac arteries in 4 pregnant patients aged 29, 32, 37 and 41 years. The combined procedures were indicated in two patients with prenatally diagnosed placenta previa (placenta praevia centralis) and in two patients with placenta praevia and suspected placenta accreta (placenta praevia accreta centralis). Fogarty embolectomy catheters were used for occlusion of internal iliac arteries inserted by cross-over technique.

Results: The insertion of Fogarty catheters was technically successful in all patients. Fluoroscopy times required for the insertion of catheters were 2.01, 2.13, 2.28 and 2.40 minutes, the radiation doses were minimal. Hysterectomy was not performed in any patients. Complications related to the interventional radiology were not found in any patients.

Conclusions: A prophylactic occlusion of internal iliac arteries is a simple method with minimal risk of complications.
