SAPHO syndrome is one of the rheumatic diseases. Its expression is quite variable. Diagnosis, especially in the early stages, is complicated. In differential diagnosis is necessary to exclude severe pathological units. We present a case of our patient, who attended several times our hospital with recurrent troubles and has been repeatedly investigated with different results. The diagnosis of SAPHO was first defined based on CT, taking into account the development of the disease as a differential diagnosis to the well known seronegative spondyloarthritis.
