Spontaneous pneumothorax remains a significant health problem. However, with time, some improvements have been made to knowledge of pathogenesis, diagnostics and both conservative and surgical approaches to treatment. The incidence of spontaneous pneumothorax in patients with normal visceral pleura is very uncommon. Rupture of a subpleural bleb or bulla is thought to be responsible in many cases. They may develop as a result of pulmonary barotrauma, typically in divers or air travellers. We present a patient with spontaneous pneumothorax who put herself at risk of pulmonary barotrauma by free-diving and frequent air travelling.
