Clinically significant involvement of respiratory system in patients with inflammatory bowel disease (IBD) is in comparison with other extraintestinal involvement very uncommon. The most common manifestation in respiratory system is chronic bronchitis, subglottic stenosis, bronchiectasis and bronchiolitis. In our case, we would like to show a patient with ulcerative colitis with cough, dyspnoea, and blood in stool. CT scan showed thickening of the right main bronchus with a consolidation in the right lower lobe. Even though the lung involvement in IBD is very uncommon, we should keep it in mind. An early treatment leads to a fast improvement of the patient's symptoms.
