In this article, we introduce less-known features of white matter lesions in patients with multiple sclerosis, which can be identified on MR sequences sensitive to changes in local susceptibility. The central vein sign indicates a finding of a vein passing through a white matter lesion and can be found in more than half of the white matter lesions in patients with multiple sclerosis. The iron rim sign is a thin annular rim of a white matter lesion caused by accumulation of iron laden macrophages and microglia, typically in chronic-active lesions.
