The appropriate choice of imaging modality has a major impact on diagnostic accuracy, risk stratification and optimization of therapeutic management in patients with multiple myeloma. Conventional radiography is no longer recommended as the first-line method, with whole-body low-dose CT now established as the standard initial examination. Magnetic resonance imaging and hybrid techniques are of particular importance in assessing bone marrow infiltration, extramedullary involvement and treatment response. The recently introduced photon-counting CT technology offers the prospect of further increasing sensitivity without additional radiation burden. This review aims to summarize the current imaging modalities and their respective contributions to the assessment of multiple myeloma, while also outlining the current diagnostic algorithm recommended in cases of suspected multiple myeloma.
