Magnetic resonance imaging (MRI) is the imaging modality of choice for the evaluation of the temporomandibular joints due to its ability to provide detailed visualization of soft tissues and functional joint dynamics without the use of ionizing radiation. The article describes the specific anatomy and biomechanics of the temporomandibular joint and the standard imaging protocol, which includes examinations in both closed- and open-mouth positions. The main pathological conditions comprise disc displacement (with or without reduction), degenerative changes including osteoarthritis, disc perforation and adhesions, inflammatory processes, as well as traumatic and neoplastic lesions. MRI findings enable differentiation of various types of disorders, assessment of their severity, and monitoring of complications such as pseudodisc formation or ankylosis. Thus, MRI occupies a key role in the comprehensive diagnostics of temporomandibular joint disorders and contributes to the optimal choice of therapy.
