Self-expandable nitinol stents placement in failed infrapopliteal angioplasty: one year follow-upOriginal article

Aim: To evaluate if stent implantation in infrapopliteal arteries can be used as bailout procedure in a case of failed PTA.
Method: Infrapopliteal PTA was performed in 47 patients/limbs, 94 arteries with chronic critical limb ischemia. All lesions were type C and D according to TASC classification. The group was composed from 37 males, 10 females, av. age 63.4 (42-82) years, DM present in 93.6 % of patients. Only ATA and ATP ("palpable arteries") were evaluated. Stents were placed in 16 arteries where PTA failed (Xpert stent, Abbott); in 54 patients "plain" PTA was performed, and 24 arteries with no stenosis at the time of PTA served as a sort of control group. Twelve months patency rate was evaluated according to palpation, ABI and Doppler US.
Results: In all cases stent placement restored the flow in the artery immediately after PTA.
Twelve months FU: non-stent group: 82% patency rate, stent group, 78% patency rate control group, 69% patency rate.
Conclusion: Stent placement in the case of infrapopliteal PTA failure changes technical failure to success and restores flow in dilated artery. At 12 months FU the patency rate of infrapopliteal arteries stented for PTA failure did not differ neither from non-stented arteries with optimal PTA result, nor from "control" group of nondilated arteries.
