Downstaging of hepatocellular carcinoma before liver transplantationOriginal article

Aim: Presentation of our experience with downstaging of hepatocellular carcinoma (HCC) in patients originally exceeding current liver transplant criteria.

Method: Between 2013 and 2017 thirteen patients with HCC exceeding Milan and UCSF liver transplant criteria were treated by transarterial chemoembolization (TACE) as downstaging method. Superselective embolization with drug-eluting beads was performed in all patients, doxorubicin loaded DC beads 100-300 μm were applied to tumor feeding artery.

Results: Downstaging was successful in all patients. Tumor response to embolization was evaluated by modified RECIST criteria on CT, all patients met Milan criteria and joined the waiting list for transplantation. Ten patients were successfully transplanted.

Conclusion: Drug-eluting bead TACE is acceptable method to achieve successfull downstaging of HCC to Milan criteria in selected patients.
